Student Registration Instructions

The listed documents below must also be provided

Partial registration packets cannot be accepted, all required documentation must be provided at the time of registration submission. 

  • Original Birth Certificate or Passport
  • A copy of their most recent Immunization Record
  • A copy of their most recent Physical
  • Valid Government Issued Photo ID of Parent (example: driver's license)
  • A copy of your Residency Information, only *1* of the following is needed
  • *Utility Bill* most recent with name and address 
  • OR
  • *Lease* 
  • OR
  • *Purchase and Sales Agreement* executed with signatures of all parties. This document is used after taking residency when a utility bill is not available.
