From the Health Office: Emergency
Contact Forms went
home at the beginning of school last week. Completed forms must be returned to school by Friday,
September 18th.
Last year’s forms will be disposed of by shredding at that time. Each student must have a
new form completed every school year. If you aren’t sure whether you sent
one in, please feel free to contact the Health Office via phone or email to
check. Thank you for your
assistance in this safety measure.
Attention
4th Grade Parents- In
keeping with Massachusetts regulations, we must have on file in the Health
Office a copy of each student’s physical exam that is completed during their
4th grade year. A
physical performed/dated April 2009 through the 09-10 school year is
acceptable. Please forward a copy of your child’s 4th
grade physical to the Health Office after the pediatrician visit this year. |
